What could possibly go wrong?

I saw this strip a while back and thought it was worth saving:


Nothing particularly remarkable about it: It's a NetJets Falcon 2000 going from Los Angeles to Monterey, California; proposed departure time of 1750 GMT (10:50 AM PDT) and a requested cruising altitude of 22,000 feet.

Nothing remarkable, that is, unless you happen to glance at the remarks section:
